Upon
placement through AtWork Staffing Services, I understand
that AtWork Staffing is my employer and will
be responsible for providing me with workers’ compensation
coverage, unemployment insurance, and overtime pay as
provided by law. I will read all safety materials
provided and be responsible for educating myself in
the safety policies and procedures followed by AtWork’s
clients where I am assigned to work. Should I incur
an injury while on a job assignment for AtWork,
I will contact an AtWork representative immediately
and report the accident (unless such injury is an emergency
in which case I will contact AtWork as soon as
possible after emergency treatment). I understand that
failure to report an on-the-job injury or use of unauthorized
treatment will jeopardize my claim.
I
certify that answers given on this application
and the information on my resume and/or any other document
used to secure employment through AtWork are true,
correct and complete to the best of my knowledge
and without mental reservation of any kind whatsoever.
I certify that I have not knowingly withheld any information
that might adversely affect my chances for employment.
I also certify that I have personally completed this
application. I understand that any false statements,
misstatements of material fact, or omissions of fact
on this or any other document will constitute grounds
for rejection of my application or, if I am employed,
may be subject to dismissal regardless of the time elapsed
before discovery.
I
authorize AtWork to investigate my work record, education,
references, and other matters related to my suitability
for employment and, further, authorize the references
and prior employers to disclose to the company information
related to my work records, without giving me prior
notice of such disclosure. I also release my former
employers and all other persons and/or companies relating
to this employment investigation from any claims or
liabilities arising out of or in any way related to
this investigation or disclosure.
I
authorize AtWork Staffing Services to obtain and release
a criminal record check as part of the employment
process. I agree to submit to drug testing (urinalysis/blood
sample screening) and/or a pre-employment physical
as part of employment process. I understand and agree
to the release of any of these test results to a company
evaluating my application with the understanding that
the results may be used to make a decision affecting
my employment status. I am aware that any offer of employment
is contingent upon the successful completion of these
assessments and failure to meet minimum standards may
result in immediate suspension or termination of employment.
I
understand and acknowledge that any relationship with
this organization, is of an “at will” nature,
which means that nothing contained in the application,
or conveyed during any interview which may be granted,
or during my employment, if hired, is intended to create
an employment contract between me and AtWork Staffing
Services. I understand and agree that if I am employed,
I may resign at any time and AtWork may discharge me
at any time, with or without prior notice, and with
or without cause. I understand and agree that no promises
or representations contrary to the foregoing are binding
on AtWork unless a specific document is authorized in
writing by me and AtWork Staffing Services’ President.
I
agree to not solicit nor accept employment with
an AtWork Staffing Services client to whom I am referred
for a full year without the express permission
of AtWork Staffing Services. Should I accept work
with a client that I was referred to through AtWork,
I am aware that the client is obligated to pay all appropriate
fees until I have worked through AtWork, at their location,
a minimum of 520 hours.
